Team Forms

Start a Team Request – This is the first step in getting started as a new NGC team. 

Team Information Form (NGC user ID required) – Registers you as an official NGC team.  We'll send your Team Box shortly after you submit your team information.  

Team Member Registration Form – Make sure all your members are registered with NGC so they can receive, information, updates and opportunities for scholarships and travel.

Global Project/F.A.CE. Goals Form (NGC user ID required) – Select an NGC Global Project and take action through fundraising, advocacy, and community education. 

End-of-Year Team Update (NGC user ID required) – Submit your update April - May to share your accomplishments and lessons learned.  Again, Access to this form is password protected.

Donor Form – To track funds submitted to NGC, we ask all teams to send this form along with all Global Project funds.  And remember 100% of funds raised goes directly to the grassroots Global Project.
